As you grow older, you may find that you need extra support to continue living at home. This is where a Home Care Package can help.

A Home Care Package is a government-funded program that allows older people to receive care services in their own home, so they can live independently for as long as possible. It can be used to receive all kinds of support, such as help with housework, transport to appointments or personal care.

A Home Care Package gives you choice and control over your care. Once you have received a package, you are free to choose a care provider you like, and work with them to create a care plan that best meets your needs. The government then pays that provider a subsidy to deliver services in your home.

We provide the following services to patients of varying ages:

Disability Care

This is a personal care service that is accessed through NDIS. Patients under this care umbrella can receive ongoing medical related care with a trained professional, as well as assistance with daily living, incontinence management, health maintenance and advice and assistance with your care. We also provide social and community engagement opportunities and activities.

Nursing And Wound Care

We can help with the ongoing medical management of injuries and conditions which cause wounds, such as diabetes and medical conditions which lead to frequent wounds and infections. This care provides services, maintenance and equipment needed to assist in the management of these types of wounds to prevent complications and induce healing.

Home Maintenance

These services involve the care and maintenance of the home, both the interior and exterior. This can include cleaning, yard work, landscaping and general repairs. Our trained contractor is ready to provide whatever services you need to keep your home safe and secure.

Emergency Care

This covers situations in which there is little or no notification for a late call-in or overnight stay. We’ll provide a trained and licensed nurse in these instances as an emergency service.

Psychosocial Care

We will work with your health team to assist in your psychosocial, behavioural treatment and the monitoring of your success. We’ll work with your team to ensure your needs are met and that your progress is recorded for future analysis and treatments.

Meal Prep & Delivery

We’ll work with your speech pathologist and dietician to provide meals that meet your exact nutritional and textural requirements in a cost-effective way. We offer free delivery for orders that are 5 meals and more. Our team will create your customised meals and bring them right to your door.
